Terms And Conditions:

• Patients have to bring their original Identity Proof like Pan card, etc. for cross verification.

• All BPL patients will have to produce required Original documents like BPL card (Yellow Card), Income certificate, etc. for availing the Facility.

• All the Free Patients will be scrutinized first on Merit.

• At any given point in time, no two offers can be clubbed together.
